Due to the popular demand of the energy rebate program, AHFC has set up a centralized waiting list. If you wish to participate in the program, contact them at 1-877-AK-REBATE (1-877-257-3228) to be placed on their list.
Once CIS has contacted you from the AHFC list, please fill out this form completely. This information will be used to complete AHFC forms and to schedule your appointment. Homeowner's information must be the same as the proof of ownership documents. Forms that need to be corrected or replaced will incur a $50.00 fee.
